Please use this form to submit your request for an in-kind donation from the LA Kings for your fundraising event. Please note the following disclaimers:

Donations are limited to one request per organization within a 12 month period.

In line with our mission statement, we aim to support Los Angeles based charities.

All donation requests MUST be submitted at least 60 days prior to the date of the event.

Please fill out this form in its entirety before submission for consideration. No written, phone, fax or email requests will be accepted.

Please do not submit the form more than once for a single event.

You must submit an official donation letter with the date, description and relevant details of your event.

If your request is approved, the items donated will be at the sole discretion of the LA Kings. We can not guarantee certain items will be included in your donated package.
